This role serves as the primary catalyst in identifying and facilitating change in the form of industry standard evidence-based best practice improvement opportunities that are required to accelerate the achievement of goals related to clinical operations excellence and optimal, sustained patient care by building collaboration, quality, efficiency and value into a dynamic healthcare delivery system.  The Clinical Transformation Consultant will join the Clinical Transformation Team to assist in building better care pathways that directly improve the overall health of patients.

Job Summary:

The Clinical Transformation Consultant (CTC) is responsible for the ongoing clinical operations management of physician practices participating in Optimum Physician Alliance’s IPA, Value Based Programs, and Population Health Management programs. The CTC will lead multi-disciplined practice transformation and practice performance improvement initiatives. The selected candidate will guide practices in achieving targeted goals that include improved quality, efficiency and utilization. The CTC is accountable for driving practice progress toward desired transformational change and performance improvement, while meeting savings goals set for the practice.

 

Key Responsibilities:

  • Accountable for successful deployment of programs at the practice level, including but not limited to, introducing and educating practices on the value and use of reporting tools, patient registries and delivery of reports via portals for enhanced patient care
  • Build and effectively maintain relationship with the practice leadership and key clinical influencers actively involved in practice transformation
  • Regularly facilitate efficient, effective practice improvement meetings with the practice to monitor, present, and discuss progress on the transformation action plan and achievement in milestones
  • Develop strategies based on practice assessment and performance analysis, for improvement that includes specific outcomes and metrics to monitor progress to a goal and make recommendations for improvement
  • Design practice transformation action plans and implement appropriate performance improvement initiatives designed to assist the practice in achieving contractually required transformation milestones
  • Monitor and review the progress of the practice in milestone achievement and insure the practice is accountable for successful completion
  • Use data to analyze key cost, utilization and quality data and interpret results to assess the performance of the practice
  • Use data to analyze trends and work with stakeholders to agree on and implement proactive strategies to address issues, and measure impact using a PDSA rapid cycle improvement approach; including external practice data
  • Assist practices in creating work flows to optimize care delivery, introduce best practices improvements, and evaluate outcomes using rapid cycle improvements PDSA methods to reach mutual goals
  • Educate and utilize technology tools to support Practice Transformation via Health IT Tools
    Integrate technology tools into practice workflows
  • Build and effectively maintain relationships with team members across both the community and the IPA
  • Consult and partners with internal partners and the practice to identify organizational and structural challenges hindering achievement of desired program outcomes
  • Collaborate with additional teams and key strategic partners in support of integrated care for our members with practices and hospitals
  • Consult and guide the practice on developing innovative solutions, including bringing best practice experience and connecting practices with other high performing practices to spur innovation
  • Work with the practice and Optimum Physician Alliance to align and integrate Optimum Physician Alliance’s care/disease management programs and population health services to augment the practice’s capabilities, support their care management efforts and add value to the patient care experience
     

Education/Experience required:

  • Bachelor’s degree required, preferred Nursing, Health Science, Public Health or related field
  • At least 2 - 3 years of experience working with providers within a medical, hospital or managed care environment required. Practice office experience preferred.
  • At least two (2) years of clinical experience and workflow design, build, test, train and optimization experience required
  • PCMH CCE and/or Certified Practice Facilitator preferred
  • At least two (2) years of experience managing process improvement and change initiatives required

  
Required Knowledge/Skills:

  • Solid understanding of health care industry, provider community and a comprehensive knowledge of medical office flow, especially related to NCQA, PCMH, MACRA, CPC+, HEDIS and regulatory reporting
  • Exhibits strong group facilitation skills and ability to interact with diverse teams
  • Excellent verbal and written communication skills coupled with an ability to create and present effective and persuasive public presentations
  • Ability to work in a changing, fast-paced, collaborative environment, must be self-directed
  • Ability to work well under pressure, manage multiple assignments simultaneously while maintaining quality standards and meeting assigned deadlines
  • Valid driver’s license and acceptable driving history required for local travel involved with position. (DMV check will be conducted). Out of state travel may be required
  • Ability to juggle competing priorities with competing priorities with proactive problem solving skills
  • Strong in the ability to support others operational success
  • Maintains appropriate confidentiality in working with stakeholders
  • Strong emotional intelligence and adept at relationship building
  • Proficiency with MS Word, Excel, PowerPoint, Microsoft Outlook, and willingness to adapt to new software
