Quality Performance Consultant

An integral part of the Clinical Transformation team, the Quality Performance Consultant is a catalyst in facilitating change through the use of industry standard evidence-based best practice improvement opportunities that are required to accelerate the achievement of goals related to clinical operations excellence and optimal, sustained patient care by building collaboration, quality, efficiency and value into a dynamic healthcare delivery system.

The primary purpose of the Quality Performance Consultant position is to provide consistent and frequent communication to our Physicians and office staff around our Quality initiatives and programs as well as collaborating with them around implementation of new projects and initiatives.

 

Job Responsibilities:

  • Accountable for driving practice progress toward desired transformational change and performance improvement goals
  • Build and effectively maintain relationships with the practice leadership and key clinical influencers actively involved in practice transformation
  • Participates on QI committees structured to achieve outcomes success and completes committee documentation that is consistent in meeting regulatory and accrediting body requirements.
  • Works professionally and efficiently with all functional areas in meeting organizational and departmental goals
  • Educate and utilize technology tools to support Clinical Transformation via Health IT Tools
  • Build and effectively maintain relationships with team members across both the community, ACO/IPA and the entire network
  • Collaborate with additional teams and key strategic partners in support of integrated care for our members with practices and hospitals
  • Work with the practice and Optimum Physician Alliance to align and integrate Optimum Physician Alliance’s QI programs and population health services to augment the practice’s capabilities, support their care management efforts and add value to the patient care experience
  • Initiate and provide ongoing education to offices around new and existing Quality programs
  • Build strong relationships with Physicians and their support staff by conducting routine meetings and providing constant communication on Quality Initiatives
  • Lead the implementation of new Quality Initiatives to all GPPC offices impacted
  • Develop strategy and rollout plans that support a successful implementation of new Quality Initiatives
  • Perform follow-up analysis for all Quality initiatives rollouts to ensure success which includes gathering feedback, adjusting workflow as needed and identifying additional educational opportunities
  • Provide executive summary of Physician feedback to appropriate Leadership and Quality team
  • Identify and present solutions as issues are identified
  • Assess current office workflows and identify opportunities for improvement
  • Maintain knowledge of current Healthcare Payment models and Incentive programs
  • Delivery exemplary customer service to provide a positive experience across the organization
  • May perform quality assessments to extract historical data from offices during mergers and acquisitions
  • Perform other duties as assigned by management


Qualifications:

  • Bachelor’s degree in Business, Nursing or Healthcare Related Field
  • 5-7 years in the Healthcare field with a Quality focus
  • Knowledge with PCMH, CPC+, MACRA/MIPS, HEDIS Measures, Quality Improvement Activities
  • Lean Six Sigma Certification – preferred
  • Strong communication skills

  • Strong presentation skills
